Instructions for Completing the PCBDD Employment Application

  1. Employment applications are available for pickup at PCBDD Human Resources, 2606 Brady Lake Road, Ravenna, OH 44266 between 8:00 a.m. — 4:00 p.m., or you may download the application, complete it using the following the instructions, print it out and submit it as indicated in No. 7 below.
  2. Please complete all sections and print or type clearly.
  3. A PCBDD employment application form must be submitted for each position for which you are applying.
  4. Please indicate the title of the position for which you are applying.
  5. Do not use “See Resume” on any part of the application. You must write or type in all requested information. You may insert a copy of your resume to provide additional information if you like.
  6. All applicants must complete the driver’s license section and indicate the issuing state, license number and expiration date, as well as initial the block at the bottom of that page.
  7. All completed employment applications and accompanying materials must be received by the deadline date for consideration. You may send your application by facsimile to (330) 297-1202, bring it in person between 8:00 a.m. – 4:00 p.m. Monday – Friday, or mail to PCBDD Human Resources, 2606 Brady Lake Road, Ravenna, OH 44266.
  8. You must demonstrate on the application how you meet the minimum qualifications for the position. There is a section on the last page where you can describe additional experience and/or training, etc., if necessary.
  9. Do not forget to sign and date the application.
  10. You may be refused employment consideration if the application is not filled out completely and accurately.
